Terms of Use

These Terms of Use ("Terms") govern your use of Symptom Tracker: Health Diary ("Health Diary", "the app"), published by SELECTA LAB LLC, Ukraine ("we", "us"). By downloading or using the app you agree to these Terms. If you do not agree, do not use the app.
License and permitted use
Subject to these Terms, SELECTA LAB LLC grants you a limited, non-exclusive, non-transferable, revocable license to use Health Diary on your iPhone for your own personal, non-commercial use, under these Terms and the Apple Media Services terms.
You may use the app to keep a diary of your mood, symptoms, medications, habits, sleep and other notes; review your history and trends; see patterns in your own entries; set reminders; export your data as a CSV file; and, with Premium, create a doctor report as a PDF and share it where you choose.
Premium subscriptions and in-app purchases
Health Diary offers an optional auto-renewable Premium subscription through the Apple App Store. Premium unlocks features such as the full list of "What affects you" findings and next-day effects, trends over 90 days and all time, comparing two metrics, detailed reports, the doctor report, and unlimited habits and custom factors. The features listed in the app when you subscribe are what is included.
Subscription options:
- Premium Monthly: one month, US price USD 7.99 per month.
- Premium Yearly: one year, US price USD 39.99 per year.
Prices in other countries are set by Apple and shown in the app in your local currency before you subscribe. The app may also show a one-time introductory price for the yearly plan; the price, the length of the introductory period and the renewal price are shown before you confirm.
A 3-day free trial applies only when the App Store offers it to your Apple ID; this is shown in the app before you confirm. Unless you cancel at least 24 hours before the trial or the current period ends, the subscription renews automatically and your Apple ID is charged the price shown, within 24 hours before the end of the period.
You can manage or cancel your subscription in your App Store account settings. Deleting the app does not cancel it. Cancelling stops future renewals, and you keep Premium until the end of the period you paid for. Refunds are handled by Apple under its policies. "Restore purchases" in the app restores a subscription bought with the same Apple ID.
Logging, medications and reminders, Export CSV, Clear all data, Help now, Restore purchases and these legal texts stay available without Premium.
Prohibited use
You agree not to use the app unlawfully, copy, resell or reverse engineer it, interfere with its operation, or bypass subscription checks, except where the law gives you these rights.
No medical advice
Health Diary is a personal self-tracking journal. It is not a medical device and does not give medical advice. It does not diagnose, treat or detect any condition or episode, and it does not replace your doctor.
The patterns, trends, findings, reports and the doctor report are calculated from your own entries. They show how your notes relate to each other, not what causes what, and they can be incomplete or wrong. Talk to a qualified health professional about your health, symptoms and medications, and do not start, change or stop a treatment because of something in the app. See also the Disclaimer in the app.
In a crisis
The app does not watch your entries, does not send alerts and cannot call for help on your behalf. If you are in crisis, think you may harm yourself or are in danger, call your local emergency number now. In the United States you can call or text 988 (the 988 Suicide & Crisis Lifeline), free and at any time; the Help now screen in Settings opens the call or the text message for you. Outside the United States, call your local emergency number.
User responsibility
You are responsible for what you enter and for checking your entries, reminders, reports and any file you export before relying on them or sharing them. Entries can be missing or wrong if information is inaccurate or deleted. When you share a CSV file or a doctor report, you choose where it goes and you are responsible for that disclosure. Your data is stored only on your iPhone, so keep your device secure and make your own backups if you want them.
Notifications and availability
Reminders are local notifications and depend on your iPhone, iOS settings (such as Focus and notification permissions) and battery conditions. They can be late, limited or missed, so do not rely on them alone for medication or anything time-critical. We do not guarantee uninterrupted availability of the app.
Intellectual property
The app, including its design, branding, texts and code, belongs to SELECTA LAB LLC or its licensors and is protected by law. No ownership rights are transferred to you beyond the license above. Your diary entries remain yours.
Disclaimer of warranties
To the maximum extent permitted by law, the app is provided "as is" and "as available". We make no warranty that it will be uninterrupted, error-free or accurate, or suitable for any particular purpose, including the accuracy of trends, findings, reports and reminders.
Limitation of liability
To the maximum extent permitted by law, SELECTA LAB LLC is not liable for any indirect, incidental, special or consequential damages, for loss of data, or for health-related decisions you make, arising from your use of the app. Nothing in these Terms excludes liability that cannot be excluded by law, including your mandatory consumer rights.
Suspension, updates and changes
We may update, change, suspend or discontinue the app or its features, and we may update these Terms. The effective date above shows the latest version. If you keep using the app after a change, you accept the updated Terms.
Governing law
These Terms are governed by the laws of Ukraine, except where mandatory consumer protection laws of your place of residence require otherwise.
